Pregnancy is often described as one of life's most joyful experiences, but that doesn't mean it is always easy. Growing a tiny human puts the body through enormous change, and those changes can affect almost everything from energy levels and digestion to sleep, emotions, and even how your body feels day to day. Many people know about morning sickness, but pregnancy symptoms go far beyond nausea.
In What No One Tells You, psychiatrists Dr. Alexandra Sacks and Dr. Catherine Birndorf explain that the emotional transformation of becoming a mother is far from a seamless switch. The authors compare matrescence (the transition into motherhood) to adolescence. Just as teenagers go through puberty and experience dramatic changes in their bodies, hormones, emotions, and sense of self, pregnant and postpartum women can go through an equally significant transformation. Their bodies change, hormones surge, and their identities and priorities may shift in ways they never expected.
With so much happening at once, it’s no wonder pregnancy can sometimes feel like a bit of a roller coaster. Some changes are widely discussed, while others can catch expecting parents completely off guard. So, let’s take a closer look at some of the most common (and sometimes surprising) changes people can experience during pregnancy.
One of the most common changes is frequent urination. As pregnancy begins, hormonal changes increase blood flow to the kidneys, which causes them to produce more urine. As the uterus grows, it can also start putting pressure on the bladder, meaning many pregnant women may find themselves making far more trips to the bathroom than usual. It may be inconvenient, but it is a very common part of the body's adjustment to pregnancy.
Fatigue is another symptom that can make the early weeks surprisingly difficult. Some people feel completely exhausted, while others experience very little tiredness at all. A rapid rise in progesterone plays a major role, as this hormone supports the pregnancy but can also have a sedating effect, leaving someone feeling unusually sleepy or sluggish. At the same time, the body is working hard to build the placenta and support the developing fetus, which increases its energy demands. As the pregnancy develops, glucose and other nutrients are increasingly directed toward supporting the growing baby, leaving the mother with fewer energy reserves for everyday activities.
Then comes the second trimester, which generally covers weeks 13 through 27. For many people, some early pregnancy symptoms may begin to ease during this stage, particularly nausea and extreme fatigue. But that does not necessarily mean the body gets a break. As the uterus continues to grow and the body produces more blood and retains additional fluid, swelling can develop, especially in the feet, ankles, and sometimes the hands. Gravity can make this more noticeable because fluid naturally collects in the lower parts of the body. Standing or sitting for long periods can worsen swelling, and heat can also contribute. Although mild swelling can be a common part of pregnancy, sudden or severe swelling should not simply be ignored, as it can sometimes be a sign of a more serious problem and should be discussed with a healthcare professional.
Constipation can also become an uncomfortable part of pregnancy. Hormonal changes, particularly rising progesterone, can relax the muscles in the digestive tract and slow the movement of food through the intestines. As the pregnancy progresses, the growing uterus can also place additional pressure on the bowel, making things even more difficult. Some pregnant women may also find that iron supplements, which are commonly recommended during pregnancy, contribute to constipation. This combination can lead to bloating, discomfort, and the need to strain during bowel movements. Staying hydrated, eating enough fiber, and following medical advice can help some people manage the problem, although anyone experiencing persistent or severe symptoms should speak with their healthcare provider.

Lower back pain is another common complaint as pregnancy progresses. As the belly grows, the body's center of gravity gradually shifts forward, which can change the way a person stands and walks. To compensate, the lower back may experience additional strain. Pregnancy hormones also loosen ligaments and joints in preparation for childbirth, which can reduce the body's usual stability and contribute to discomfort. A growing uterus, increased weight, posture changes, and the physical demands of pregnancy can all play a role. For some women, the pain may be mild and occasional, while for others it can make sleeping, standing, or completing everyday tasks considerably more difficult.
Hemorrhoids can also become an unfortunate part of the experience for some pregnant women. The growing uterus can put pressure on the veins in the lower body, while hormonal changes may contribute to constipation. Straining during bowel movements can place even more pressure on the veins around the rectum, potentially causing them to swell and become painful or itchy. Gravity doesn't help either. For someone already dealing with fatigue, digestive issues, back pain, and a rapidly changing body, this can be yet another uncomfortable symptom to deal with. It is a reminder that pregnancy affects far more than just the belly, and many of the changes happening inside the body can create symptoms that people may not have expected before becoming pregnant.
Of course, no two pregnancies are exactly alike. One woman may experience intense nausea and exhaustion, while another may feel relatively energetic. Some people deal with swelling, heartburn, and back pain, while others barely experience them at all. Pregnancy can be joyful, exciting, and deeply meaningful, but it can also be physically exhausting and emotionally demanding. A lot is happening at once, and the body doesn't always make the process easy.
So when we consider everything a pregnant woman may already be navigating, adding intense emotional turmoil and relationship problems on top of those physical changes can sound incredibly overwhelming.
