Oh they did the same thing with Maisie Williams and Arya. I don’t remember the exact ages but it is also a huge gap

remember when they said that they have been filming non-stop for 55 days. 55 days, at night. that'll take its toll on people. they were just all exhausted.
I've worked on sets and trust me, it's possible to miss something. There's so much going on, the crew has a lot to focus on, actors are often careless. If it's noticed in post, there's not really much they can do about it-it's not like the scene can be re-shot. All they can do is edit to the best of their ability and hope no one notices. .
